To qualify for a First Time U.S Passport:
You must be a U.S. born citizen or naturalized citizen of the U.S.
You never previously acquired a U.S. Passport.
You have had a passport but your previous passport is expired and was issued more than 15 years ago.
You have had a passport but you were under 16 when you acquired your previous passport.
You have had a passport but your passport was lost, stolen or damaged
Note: Government Rules require you must have all of the following items and appear in person at a nearest passport acceptance facility before we can help expedite your application. You will be handed a SEALED envelope. Please bring the SEALED envelope to us, we only have 5 days to submit to the U.S Passport office.
Proof of I.D (You must submit one of the following)
A state issued Drivers license of State issued I.D
Proof of Citizenship ( You must submit one of the following)
Certified copy of your birth certificate, must show full names of the applicant's parents.
Original Naturalization certificate
A previously issued expired, undamged expired passport
Two (2) passport quality photos.
Color photos, 2X2 inches in size and with a white background.
Check or money order: $170.00 payable to “ U.S. State Department - Passport Services ”.
Check or money order: $ 25.00 local office filing fee

Copy of your flight itinerary, airline tickets or cruise confirmation.
Without proof of travel, the State Department cannot guarantee issuance of your passport by your travel date.
